NOTIFICATION OF TRANSFERRING MEMBER
At the end of each term, use this Form to notify Headquarters of transfers from your Chapter to other schools. If we have a Chapter at that school, we will notify the Chapter.
A COMPOSITE of all transfer lists for the school year is to be submitted to Headquarters as part of the annual Chapter Report. If there are no transferring members, write "No Transfers" on the form and SUBMIT THIS FORM WITH THE CHAPTER REPORT.
